Tribal Dementia Care Specialist (DCS)
The Tribal Dementia Care Specialist (DCS) works in partnership with the Tribal Health and Human Services agencies and Aging and Disability Resource Centers (ADRCs) in the assigned tribes service area to ensure that the Indian Health Service (IHS) eligible individuals receive culturally appropriate information and are able to comfortably and effectively access dementia and long term care programs and services. The Tribal DCS will have three primary goals: facilitate dementia friendly community efforts, train staff in tribal departments to be dementia capable, and support people with dementia and their family members.
Engage in marketing and outreach to inform eligible individuals about services and programs available. This position will serve the Lac du Flambeau and Sokaogon tribal communities.
Provide dementia-specific consultation and technical assistance related to cognitive screening, individual and family caregiver issues, the adult protective services agency (APS), county/tribal aging offices and tribal staff members.
Provide programs that will create opportunities for tribal members with dementia to remain living safely in their own homes.
Apply cognitive screening policies and protocols. Offer and provide person-centered follow-up when screens are positive and assure that opportunities for support and follow-up with primary care provider are available in accordance with the individual’s preferences.
Provide information, assistance, and options counseling, in homes (home visits), in the ADRC office, tribal offices, or over the phone.
Develop referral relationships with physicians; dementia diagnostic clinics and other health and long term care providers.
Provide short-term service coordination to help individuals deal with immediate needs, to the extent funding will allow.
Collaborate actively and develop referral protocols with local, regional and statewide dementia organizations, the Wisconsin Alzheimer’s Institute and the statewide research centers.
Assist the tribe in implementing strategies to create dementia friendly communities in the tribal service area.
Provide volunteer training, technical support and program oversight.
Provide outreach to non-healthcare professionals, employers, organizations and the general community about the tribe, ADRC and available dementia services.
Maintain knowledge and familiarity of dementia, research findings, new evidence-based interventions for people living in the community, and other innovations for people with dementia.
Assist colleagues by sharing experiences. Provide conference workshops to increase the awareness of the program and outcomes. Provide or arrange training on cultural competence in working with tribes and eligible individuals.
Develop and maintain client records, client-tracking records, and client resource directory and database.
Maintain knowledge and familiarity of tribal resources, local area resources, and statewide resources
Provide accurate and timely 100% time reporting to capture matching revenues to support the services.
Develop and maintain culturally sensitive informational materials.
Contribute to ongoing strategic planning, reporting and program support to ensure the successful development and implementation of the program.
Assist participating tribes in strategic planning, assessment implementation and evaluation of activities and strategies.
Consult with Department of Human Services (DHS), Bureau on Aging and Disability Resources (BADR) on issues relating to the DCS services.
Actively participate in task teams, working groups, and other joint program initiatives.
Prepare, review and forward reports according to program requirements.
This is a full-time salaried opportunity with scheduled hours of Mon-Fri 8:00 AM – 4:30 PM, 40-45 hours per week with a primary work location in Lac Du Flambeau, WI. There will be variations to schedule to accommodate clients and families. Regular travel to Mole Lake Reservation will be required.
This position has a salary range of $46,000 – $50,000 with benefits including Annual/Personal Leave, Holiday Pay, Health, Dental, Life & AD&D, STD and LTD Insurance, FSA and 401(k) Retirement Plan. Background check and drug screen are required before employment.
Qualifications: Any equivalent combination of experience and education that would provide the required knowledge and abilities is qualifying. Required: High School Diploma. Additionally, a minimum of two years of paid experience working directly with multiple people with dementia and family/informal caregivers (e.g., providing direct care, caregiver support, support group facilitation, residential care management, home care.). Preferred: Bachelor’s degree in human services or related field, be licensed to practice as a registered nurse, or be a licensed health professional employed by a tribal health program. Expertise and knowledge regarding older adults, dementia, dementia care and support, family and informal caregiver needs, and challenging behaviors. Skills and experience in facilitating small group learning and discussion programs. Experience with family caregivers of people with dementia and family dynamics. Experience developing collaborative relationships, preferably with community organizations and health care professionals. Understanding of the traditional American Indian health issues. Experience working with tribal elders and/or a thorough understanding of elders’ roles in American Indian communities. Experience working with American Indian Tribal Governments, Tribal organization and/or not-for-profit agency. Ability to work independently and as a team member.
