Description
The Elders Wellness Support Specialist provides participants of the Elder and Caregiver Programs with a wide range of services and support needed to stay healthy and safe in the community. Responsible for assisting the Elders Department Director with developing and implementing programs focused on accessing health and wellbeing needs, including addressing social isolation. This position involves coordination with public health entities to prevent and respond to pandemics such as COVID-19 and prepare for future health challenges.

Principle Duties
Assist in development and implementation of health and wellness programs, education, and information to prevent disease, prolong life, and promote health; some examples include Healthy Heart, Falls Awareness, Alzheimer, and Dementia
Provide activities addressing social isolation
Identify client health and wellness needs and provide referrals to services and resources
Conduct assessments, surveys, wellness checks, and home safety visits
Works closely and collaborates with other Samish Departments to provide holistic, culturally relevant services
Plan, coordinate, and participate in Samish Citizen Health Fairs and other Samish Citizen events
Attends program trainings, webinars, and conferences as needed
Maintain accurate organized files, including intake forms and reports; compiles data for statistical purposes and required reporting
Prepare health and wellness information packets, mailings, newsletter, and website narratives
Maintains strict confidentiality and HIPPA compliance regarding all client information
Understands and complies with Adult Protective Services (APS) mandated reporting requirements for vulnerable adult abuse and neglect
Completes other duties as assigned

Required Qualifications
Associate Degree in health, wellness, or human services related fields or 3 years of experience working in a field related to health, wellness, or human services
Must have the ability to demonstrate exceptional customer service and client relation skills
Must have strong time management, organization, and communication skills
Must be detail oriented, naturally inquisitive and a proactive problem solver
Must possess intermediate Microsoft Office skills with the ability to learn data reporting software applications
Excellent problem-solving and listening skills
Support and contribute to a collaborative and respectful work environment that promotes teamwork
Must obtain First Aid, CPR and AED certifications within 30 days of hire
Must have proof of a Tuberculosis (TB) test taken within 12 months prior to initial employment date and be free from communicable disease
Must possess a valid driver’s license and insurance and be able to operate tribal vehicles and a chair lift.
**Obtaining full COVID-19 vaccination no later than 60 days from the hire date will be a condition of employment for all Samish employees**
Working Conditions
Physical Demands: Long periods of sitting (70% of time); occasional bending, crouching, stooping, stretching, reaching, lifting, carrying, and pushing (10% of time); and occasional standing and walking (20% of time). Occasionally lifts/carries up to 30 lbs and assists participants with mobility devices such as walkers and wheelchairs, requiring pushing and lifting. Must possess manual dexterity sufficient to use office equipment with frequent repetitive motion such as writing, typing, and phone use. Must possess the ability to view print on a computer screen and/or paper document; the ability to speak and hear exchanging information with end users by telephone. Mental/Visual Demands: Frequent mental and/or visual attention to task is required. Occasionally work requires heavy concentration. Frequent interruptions. Tasks involve eye-hand coordination. Working Environment: Work environment is mainly indoors with heat and some air conditioning. Must sit or stand at desk for majority of shift. Occasionally works alone without benefit of supervisor or co-worker. Unavoidable hazards/risks: Work requires minimal risks which require safety precautions (using some cleaning chemicals for light cleaning). Protective clothing or equipment may be required (goggles, gloves, masks, boots, etc.)
